A feedback form customers can comfortably finish

Review the question, fields and mobile journey so sharing an experience feels clear and straightforward.

Before sharing a form, try answering it as if you have just left your business. Do you understand the question? Do you know what will happen to your response? Can you finish without looking for instructions?

Reducing friction starts with those questions. This guide suggests a practical review; it does not promise a conversion rate or require capabilities your form may not have.

Start with a question someone can answer

Ask about a specific experience. ‘How was your visit?’ leaves room to respond; asking about friendliness, speed, price and the result together requires customers to organise too many thoughts at once.

Avoid putting your preferred conclusion inside the question. The customer may have had a different experience. A neutral question helps you understand both the positive details and what needs attention.

Question: What would you like to tell us about your visit? Optional hint: You can describe what worked well or what we could improve.

Justify each field before asking for it

Review the information you need to understand the response and what you request out of habit. More fields do not necessarily produce more useful feedback. Keep the actual requirements of your chosen form clear.

Distinguish optional fields from required ones. If you offer video, present text as an alternative where available and explain what happens before activating the camera. Do not promise anonymity if you ask for identifying information.

  • The label explains what information is requested.
  • Examples do not replace labels.
  • Requirements are visible before submission.
  • Publication permission is understandable on its own.

Try the form with one hand on a phone

Open the link on a phone and check whether the keyboard covers any fields or actions. Check that the text is comfortable to read, labels remain visible while typing and you can correct an answer without starting again.

Test the QR code where people will use it. Distance, reflections and print quality are part of the journey. If it opens an unexpected page, correct the destination before sharing it.

Finish by explaining what happened

After submission, thank the customer and make it clear whether their response was received. If an error occurs, check that the next step is understandable and that a failure is not presented as a confirmation.

If the journey offers a next step to Google, that is an additional decision for the customer. It does not mean their response has already been published there. When reviewing results, separate visits, completed responses and clicks to another destination.
